PostSubject: Application   Application I_icon_minitimeMon May 13 2013, 11:24

Good day everyone! I just wanted to take a quick moment to announce that NLPRS is now taking applications for membership!

**Keep in mind that this application is for employment within NLPRS, and not for membership to this website. Also, NLPRS is a non-paying agency.**

With an increased number of members, this would ensure greater investigation coverage. How so? Simple! Most of our members have full time jobs and / or families; we fully understand that their private lives come before an investigation. By creating a database of members and their skills, we can better determine who would work best for any given situation.

Now then, the application can be seen below. There is no need for you to post your answers here. Again, there is no need for you to post your answers here. In fact, I would rather you didn't!

There are two options available to you to complete this.
1. Email me your interest, and I shall email you the questionnaire in return.
2. Copy and paste the questionnaire into either your document reader or email provider.
Either way, just answer the questions and email me the completed form. Just a word of warning: if you copy and paste the questionnaire, you may want to change the font color. Those that email me for a form will have it sent to them as a Word Document; it will not be sent as an attachment, but rather will be included in the body of the text.

**EDIT... I figured out how to attach a document to posts on this site! You now have the option to download the questionnaire directly without having to worry about fonts or colors or copy / pasting. **

Lastly, there are some things to consider when filling this out.
1. Be honest. I really don't care what you think (well, I do, but that is not the point). I would rather you be honest about things.
2. Answer everything. Even if you answer "no" or "N/A", answer everything.
3. Upon receiving your answers, I will be contacting you. You see, the questionnaire is just the first step. We will have to arrange for a meet to see if we can work with each other or not, as the questionnaire was business, and this is personal. Admit it, this is an interest; would it help either of us of our interests were marred, or if we somehow conflicted?

Alright, here it is... our application / questionnaire. Remember, don't post your response here, but rather email it to me (my email address is located at the bottom of this post). I look forward to hearing from you!

...

BASIC INFORMATION
Name:
Email address:
City, County, and State where you live:
Age:

Do you have access to your own transportation?
Most of our members have no issue with offering or sharing a ride. However, as most of our members are scattered throughout Marinette County, it would be asked that you would at least be able to meet somewhere to carpool (if we do not have a member near your location).

Depending on the day, and if enough notice is given, would you be able to work the entirety of the day and / or evening?
Some investigations may involve over-night work, or require a long drive to reach before an investigation may begin. All of our members will be told of such events well in advance, as we understand that our private lives must come first.

Have you ever been convicted of a felony? Do you have any limitations? If so, please explain.
Be aware that being a felon does not exclude you from membership. However, there may be some investigations which we would not be able to utilize you on the field.

If requested, would you be willing to submit to a background check?

OCCUPATIONAL

What is your current occupation? What shift do you work? What days do you work?
Answering this question helps us determine what days you could be available for investigations. Because most of us holds jobs and have families, it is understandable that all members will not be available all the time.

Do you have any certifications, skills, or hobbies that might be beneficial to NLPRS?
Hobbies, skills and interests could include, but are not limited to, photography, research, AV or computer experience. When it comes to certifications, please be specific; you never know that your additional training or education might be able to provide.

Do you have any physical limitations that could hinder or impede either yourself or a team during an investigation?
Each investigation varies in scope and what an investigator may be required to do. Most investigations may require you to stand for long periods of time, and may involve walking over large distances or through wooded, marshy, or hilly terrain.

Do you own, or have access to, any equipment that you would bring to an investigation? If so, please explain.
Common items might be a cell phone, two way radios, or a digital camera. Further equipment could include a laptop or tablet, EMF meter, recorder, infra red thermometer, and so on. Possession of your own equipment is not required, but is recommended.

Some investigations may require you to sign a nondisclosure agreement, a confidentiality agreement, and / or waivers from harm. Would you be willing to sign such forms if required?

PARANORMAL UNDERSTANDING
Why are you applying for NLPRS membership?

Concerning the paranormal, how do you see yourself? Are you a full believer, a skeptic, or somewhere in between? Please explain.

Do you profess to have any paranormal related abilities? If so, please explain. If you answered yes, would you be willing to have this verified or tested (if your ability can be)?

Do you understand that a real world investigation differs from those seen on television?

Have you ever conducted an investigation, or been part of an investigation? What did the investigation involve? What was your role?

Have you ever experienced the paranormal, or have you ever witnessed something that could not be explained? If so, please explain.

Now is the time for you to tell me about yourself. Likes, dislikes, interests, whatever. Tell me why you think you would make for a good NLPRS member.

Please return the completed application to nlprs.wi@gmail.com Make sure you include the word application in the subject line!

As a reminder for those interested in joining NLPRS, a filed application is required.  This is done so we can maintain a professional dynamic; also, it allows me the chance to see where everyone is coming from (both physically and mentally), as well as giving me the information required to conduct a background check.

Before you start scratching your head or anything, this isn't a "real" background check.  I'm not looking into your work or credit history, nor am I looking to see how often you've moved in the past year.  I don't care if you've been pulled over for speeding or some other minor traffic violation like that (although severe or repeated reckless driving is something else... equipment is not going in your vehicle, and you drive yourself).  Nor do I care about a lot of what I would consider "minor" infractions.

All I am looking for are histories of theft, anger issues, repeated drug use, and crimes of a sexual nature.  There are crimes that I might apprehensive, but if you've done your time then I have nothing against you.  Keep in mind I will ask you about such things; I want to know your story, and if what you have to tell me sounds sincere, then that's one thing.  However, if there is a known and repeated history of particular crimes, or if what you say doesn't sound honest, then that would prevent further consideration.  Please keep in mind that we are either invited into someone's private home or deal directly with the public... be straight-forward and honest with me, and that'll go a long way.

Any information collected will only be seen or known by me.  Unless you ask me to see what I find, I won't tell you.  All information is kept confidential.

There are a few websites I go to to check someone's background, all of which are free and open to the public.  Again, all the sites I go to anyone can access.  I am not paying a company to dig up your past, these are free and publicly available sites.

{EDIT}

I do maintain a file on all of our members. This file holds the application, a picture of the individual, background check, and any paperwork that you would have to submit in investigations (such as disclaimers, liability issues or confidentiality agreements). I am the only one with access to these files, so no worries.
